- Sign the document:
Once you have read and understood the Code of Conduct, sign the document to acknowledge your agreement and commitment to abide by its provisions. Ensure that your signature is legible and matches the name provided in your application.
- Bring the entire signed document:
Prior to your next scheduled interview or intake, ensure that you have the complete Code of Conduct document with your signature affixed to it. Do not detach any pages or alter the document in any way.
- Submit during interview or intake:
When you arrive for your scheduled interview or intake, present the entire signed Code of Conduct document to the appropriate person or designated representative. They will collect and review the document as part of the application process.
