I hereby agree and consent as follows:
As a parent or legal guardian of minor child(ren), I certify and affirm that I have been completely and thoroughly informed that my child will participate in certain activities which carry with them a degree of risk and danger. Examples of risky and dangerous activities include but are not limited to the following: Physical activities, both indoors and outdoors; Sports, both informal and organized; Use of recreational equipment; Ministry, both on and off campus; Travel by automobile; Activities in low income areas; Evangelizing in impoverished areas; Construction and maintenance projects.
I acknowledge and understand that The Mission may offer other activities. I acknowledge and understand that this PARENTAL CONSENT AND RELEASE has the same force and effect regardless of whether the activities engaged in are free or if a fee is charged.

Further, I personally assume, on my child's behalf all risk in connection with said activities for any harm, injury or damages that may befall my child as a result of my child's participation in the activities whether foreseen or unforeseen, and I still wish to allow my child to proceed with the activities.

I understand that the terms herein are contractual and not mere recital; I have signed this document as my own free act. It is my intention by signing this document to exempt and release The Mission from all liability whatsoever for personal injury, property damage or wrongful death caused by negligence.

I further acknowledge and agree that my signature on the PARENTAL CONSENT AND RELEASE shall constitute a bar to any recovery by my child, me, or my family, heirs, or assigns in all suites and actions that may be instituted against The Mission, its agents, servants, or employees, or due to contributory negligence of my child.

I understand that it is my obligation to inform the management of The Dream Center of Columbus of any and all health considerations or medical conditions that would restrict my child's participation in any and all activities involving The Mission or its programs.

I have fully informed myself of the contents of this PARENTAL CONSENT AND RELEASE.

I am a volunteer worker and not an employee of The Mission. I am aware of the hazards and risks to my person and property associated with serving in a missions capacity, such hazards and risks including, but not limited to, death or injury by accident, disease, war, terrorist acts, weather conditions, inadequate medical services and supplies, criminal activity, and random acts of violence. I accept my assignment as a Missionary-Volunteer with full awareness of these risks, and subject to any insurance coverage that may be available to me from any source, and I voluntarily assume all risks death, injury, and illness associated with such risks, and any damage to my personal property, and I release The Mission and its agents, officers, directors, and employees from any liability whatever arising as a result of death, injury, or illness that I may suffer as a result of participation in the missions project. I further recognize that such risks have always been associated with missionary service. 2 Corinthians 11:23-28.

I attest and certify that I have no medical conditions that would prevent me from performing my duties as a Missionary-Volunteer.

I expressly waive any defense to the enforcement of any provision of this commitment arising from a claim of lack of consideration and warrant that this commitment constitutes a legal valid, and binding obligation upon me enforceable against me in accordance with its terms.

I am aware of the hazards and risks to my person associated with participation in The Mission as a Missionary-Volunteer, as described above. I further understand that The Mission may not have any insurance coverage that would apply in the event of my death, illness, injury, or damage to my property that may occur during my participation as a Missionary-Volunteer, and if I desire insurance coverage I am responsible for the cost of such insurance.

I expressly agree that this assumption of risk agreement is intended to be as broad and inclusive as permitted by law. I further state that I have carefully read the foregoing assumption of risk and understand its contents, and I voluntarily sign this release as my own free act. This is a legal document and I understand that I have the opportunity to consult with an attorney before signing it.

I hereby grant The Mission, its directors, officers, employees, agents, and designers (collectively “The Mission”) non-revocable permission to capture my image and likeness in videotapes, motion pictures, recordings, or any other media (collectively “Images”). I acknowledge that The Mission will own such Images and further grant The Mission permission to copyright, display, publish, distribute, use, modify, print and reprint such Images in any manner whatsoever related to The Mission, business, including without limitation, publications, advertisements, brochures, web site images, or other electronic displays and transmissions thereof. I further waive any right to inspect or approve the use of the Image by The Mission prior to its use. I forever release and hold The Mission harmless from any and all liability arising out of the use of the Images in any manner or media whatsoever, and waive any and all claims and causes of action relating to use of the Images, including without limitation, claims for invasion of privacy rights or publicity. I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name or, if I am less than eighteen years old, that my parent or guardian has signed this release form below. This release is binding on me and my heirs, assignees and personal representatives.
